Describe diabetes self-management education and support


Problem:

This project addresses a quality improvement and health education need related to the underutilization of Diabetes Self-Management Education and Support (DSMES) services among adults with type 2 diabetes mellitus (T2DM) in rural and underserved communities. Limited access to DSMES in these areas contributes to poor glycemic control and significantly higher hospitalization rates, as rural patients often face barriers such as provider shortages, transportation challenges, and lack of structured education programs. Addressing this need is critical because T2DM is a major public health concern, and individuals in rural communities experience disproportionately worse outcomes due to systemic healthcare disparities. Implementing a structured, nurse-led DSMES program can empower patients with the knowledge and tools necessary for effective self-management, leading to improved clinical outcomes and reduced healthcare costs. Enhancing access to DSMES not only supports better individual health outcomes but also advances health equity by closing the care gap between rural and urban populations. Several pieces of evidence support the urgency of this need. Siminerio et al. (2023) found that adults in rural areas with uncontrolled diabetes have hospitalization rates that are 20-30% higher than those in urban areas. Research by Jarl et al.
