Describe - all pain assessments are different


Problem: Respond personally in paragraph form to discussion post : All pain assessments are different. Each patient experiences pain differently. A patient with sickle cell disease may appear to not be in pain, even though their pain is a 10/10. These patients are used to dealing with chronic pain. The nurse is taught to go off the client's report and rating of their pain. I've had numerous pain assessments working as an RN. As a psych nurse, I do pain assessments every admission, during medication administration, at 10pm, and at 6am. If a patient states they're in pain and medication is given, a pain reassessment is done within an hour. The patients are asked to rate their pain on a scale of 1-10. The patients are then asked to describe their pain (burning, aching, sore, etc.) and what location of the body their pain is in. While listening to the patient's report of pain, I'm also watching for nonverbal indicators, such as body language, facial expressions, posture, and tone of voice. Need Assignment Help?
