Imagine that you now need to conduct an intake assessment at your agency for the client you identified in Part 1. Use Chapter 4 of the course text as a guideline.
1. Create a unique and detailed intake form that includes the information you think you would need to know in order to determine service eligibility and develop a service plan for the client at your specific agency. Be sure that the information requested on the intake form is relevant to the work of your specific agency.
2. Complete the intake form using information you learn about the client in the film/novel. If the information you need is not available, fill in your best guesses and inferences based on everything else you know about the client.
For example, information about the client's psychosocial history may not be available - you arc permitted to fill in details creatively.
