Shelby is a 40 year old white female seeking assistance in your office after finding out her spouse has had an affair. She is visibly shaking and sobbing at your first encounter. She expresses having difficulty completing simple tasks like getting dressed in the morning. She has a very demanding job as a nurse, taking care of others, and she can't seem to concentrate on anything. Shelby has a medical issue that made it difficult for her to work full-time and her husband carried the medical benefits for both of them. She has no children and up until this event was living happily with her husband of 16 years in Philadelphia; 2 hours away from her hometown of Scranton. Her parents are still living in the house she grew up in with her 2 siblings. She has 3 nephews she enjoys spending time with them but they also live 2 hours away. Her husband has moved out and she is alone. She cannot afford the house on her own and she has a large dog that also has medical issues that will make it difficult to find an apartment. She has not told any of her friends or family about this. She is catholic and has been talking with a priest from her church who has been trying to help her make sense of this mess. She cries daily, she cannot control her body from physically shaking, and she expressed she cannot see a future for herself
