Program Context

- Problem/need addressed: Evidence-based description of the issue (cite at least 3 sources)
Theory of Change and Logic Model Explanation
- Theory of change: Clearly articulate the program's theory of change, that is, how and why the program activities are expected to lead to the desired outcomes. Explain the causal mechanisms linking program components to expected results with theoretical basis and research support. It is essential to cite scholarly sources to support this section.

2. Logic Model Diagram (1 page)
Create a professional logic model that includes:
- Inputs/Resources: Staff, funding, facilities, partnerships, materials
- Activities: Specific program components and interventions
- Outputs: Quantifiable products of activities (e.g., # of sessions, participants served)
- Short-term outcomes (0-6 months): Immediate changes in knowledge, skills, attitudes
