Problem: Condense this Client was expelled from high school for making a bomb threat and had an IEP in 5th grade. Client struggled in school, particularly with math. In terms of relationships client shared Client can form relationships and friendships, but becomes angry when drinking, leading to conflicts with spouse. in terms of family dynamics client shared Client's parents are deceased. He gets along with his wife, kids, and other family members when not drinking. Client was recently discharged from ACCS and provided with a safety plan. During this intake assessment client denied any SI/SIB/HI/AH/VH. Protective factors include the client's awareness of his substance use problems and his willingness to seek help.
