According to the Air Transport Association, a person could fly every day for 3,859 years without being involved in an aircraft accident. That's an accident rate of one accident for every 1.4 million flights based on 2009 data.

Air travel today remains safe, thanks in part to accident investigation. Findings from accident investigators pave the way for changes to be made In aviation, such as changes to pilot duty, Maintenance requirements and priorities etc... These changes are improving technology, preventing accidents and saving lives.

In light of commercial aircraft accidents and incidents, lots of improvements to aircraft system technology have been made. These improvements have played a significant role In reducing and maintaining the aircraft Incident / accident rates globally. In this assignment you are requested to make a full study of at least 1 aircraft accident due to system failure. Your assignment will be divided in two main aspects: the Mechanical/Theoretical aspect and the actual investigation.

1) Describe the accident In full detail

• Summarize/Highlight the major points of the accident reports.
• State clearly the accident finding In the reports.
• Briefly outline the report structure of an accident Investigation.
• Evaluate the recommendations raised with the existing regulation governing the operation.
• How these specific accidents have changed civil aviation regulations globally.

2) Explain how the system should have worked in normal situations

• Discuss in details the structure of the system(s).
• Evaluate the system operation.

3) Explain the reason which led to this failure

• Identify the component(s) which lead to this failure.
• Analyse the effect of the failed component on the rest of the system.
• Evaluate the effect of the failed system on the performance of the aircraft.

4) Conclusion and Recommendation

• Improving the system operation.
• Reducing the incident/accident rate.
