This 53-year-old male patient comes to the emergency department after being found unresponsive. Patient has a known history of lung carcinoma and has been told to expect this outcome at some time in the near future.
Patient was found by paramedics in sinus bradycardia with a pressure. IV started and patient was intubated on the scene. Atropine and epinephrine were administered on several occasions without development of a pressure. Patient placed on pacemaker but no pressure or pulse developed.
On arrival, no pulse, no respirations, no blood pressure, undergoing cardiopulmonary resuscitation. Pupils fixed and dilated; corneal reflexes absent. Heart sounds not heard, no spontaneous respiratory effort. No peripheral pulses, femoral pulses felt.
