Problem
I. Risk for injury related to her disorientation and confusion. R/T Agnosia, Intervention: Promote safety is to assess the patient's patient's and remove any hazards (Vera, 2022).
II. Impaired verbal communication related to AD. R/T Aphasia Intervention: Use simple language and ask direct questions requiring short one-word answers (Vera, 2022).
III. Risk for wandering related to AD. R/T wandering to unplanned destinations Intervention: Follow routines, and determine the reason for wandering, reducing anxiety induced due to the new location (Vera, 2022).
IV. Risk for disturbed sensory perception: R/T irritability and changes in behavior. Intervention: Assess the level of consciousness and orientation to determine any changes potentially related to environmental factors.
