Problem: Eshe Kidane, a 41-year-old female client (DOB: 2/22/1983), has a history of hyperthyroidism and is at her first follow-up visit with the NP one day after being discharged from the hospital after a total thyroidectomy. The surgical site does not have any bleeding, drainage, or hematoma, and the client's physical exam is otherwise normal. Labs outside of thyroid function were normal yesterday at the hospital. While the client was taking methimazole (Tapazole) 10mg PO daily prior to surgery, it was discontinued the day of surgery. The NP considers appropriate thyroid hormone replacement medications since the client no longer has a thyroid. The provider also orders calcium and magnesium lab tests as high levels often occur after surgery and need to be monitored, and educates the client on symptoms of hypercalcemia and hypermagnesemia.

Past Medical History: Hyperthyroidism (Grave's Disease)

Allergies: None

Medications: hydrocodone 5mg/acetaminophen 300mg (Vicodin) PO PRN Q4-6 hours as needed for pain

Social History: She has never smoked cigarettes and does not drink alcohol.
