You are an assistant manager at Colesworths, Bondi Junction. Colesworths is Australia's largest supermarket chain. Over the last few years the company has been dismissive about the importance of WHS. This changed a month ago when both an employee and customer were seriously injured when the worker, attempting to put a heavy box on a top shelf, fell off the ladder on to a customer. The company's CEO has requested that each store delegate an assistant manager to administer the store's WHS policies. The CEO has asked that each store submit a report detailing the provision of WHS training, procedures and legal requirements for identifying hazards and procedures for maintaining WHS records.

It has been a particularly busy day in your store and you decide to conduct a hazard identification walk. You take the following notes:

The front of the store around the checkouts is well maintained and tidy, however one of the checkout staff is rocking back and forth on their chair, due to an uneven surface. Aisle 6 has milk spilt on the floor, which would appear to be there for a period of time, as it has spread up and down the aisle. A box has been left unattended on top of a ladder, probably the result of a worker assisting a customer. The door leading to the administration offices, including the cash office has been left open.

It is 10 pm and one of the casual workers, Mark Waugh, is stocking shelves on the 6 to 12pm shift where you are the shift supervisor. Mark has been with the company for over 12 months. Mark is carrying a box containing glass containers of jam, and slips on a puddle next to the freezer section. The smashed glass containers he was carrying cause a cut on one of his hands, and his wrist is badly swollen in an attempt to break his fall. Steve (his brother) is a part time worker working the same shift and saw the accident happen.
