3. Complete an incident report on what you observed in the scenario video. You may use an incident report form from your employer. OR, you may find a template in Word or online.
Be sure on the incident report form you include:
- The names of people involved
- Your direct observations
- Details as to time and place
- At least two strategies that could be considered to prevent this type of incident from occurring again
