Addressing patient introduction and patients background

Assignment Task: To write this assessment, you will utilize the clinical reasoning process, and address the following sections:

1. Patient presentation

In this section, you are expected to address: (1) Patient introduction, (2) patient's background, and (3) patient's situation, which includes presenting some of the key clinical findings and the timeframe of the events. (Approximately 100-150 words).

2. Collect cues

Presenting in a table format the most current patient's findings by applying A-E primary assessment framework with all the abnormal findings highlighted and normal range values providing in-text references.

3. Process information

Describing the processing of information. In this stage, you have to analyze the cues and distinguish between the relevant and irrelevant information. Cluster cues together and identify relationships between them. Match the clustered clues with relevant patient presentation, past situation (if applicable), and describe the Pathophysiology of the patient's condition. (Approximately 500-600 words including in-text citations).

4. Identify critical problems

Prioritize three relevant critical problems for your patient scenario. You can opt to use Medical terminology and/or Nursing diagnosis framework.

5. Identify interventions

Presenting a plan of care for the identified critical problems with appropriate and specific interventions. Identify two priority interventions for each critical problem (6 in total).

6. Explain the rationale behind the interventions

Explaining why your interventions will prevent deterioration in the patient's condition. Describe why you chose these interventions (one or more). The rationale for the interventions has to be supported by the underlying Pathophysiology and evidence-based literature. (Approximately 50-100 words for each rationale).

7. Detail the outcomes

In this section the interventions for the patients are evaluated. What do you expect the care provided to have on the patient's condition? How would this be measured? How would you expect the patient to respond if your intervention is working?

8. Reference List

Note: Total 2500 words.
