Pick a topic and come up with a clear research question you would like to address through your survey. Keep in mind that the data you collect will be limited so your question should probably be fairly simple and descriptive in nature.
Generate five close-ended questions that can help address address whatever question it is you're interested in. These are questions in which you provide the response options and participants choose a response. For example, this can be rating agreement on a Likert scale (e.g., from 1 = Strongly Disagree, to 5 = Strongly Agree), or a set of nominal options, like a multiple-choice question (e.g., "Basketball," "Baseball," "Football," "Soccer," "None of the above."). Please take extra care to avoid any questions that could be interpreted as remotely controversial or offensive
Generate one open-ended question that addresses your question of interest. For open-ended questions, you do not provide the response options, but instead people can write in whatever they like as a response. A good open-ended question will generate a long response and some complexity in response, rather than a simple "Yes," "No," or "Sometimes." You will have to create a coding scheme to try to categorize these open-ended responses, so you want to make a question that is likely to relatively detailed and nuanced responses, that you can then code into different categories. As an example of a poor open-ended question, consider "Do you like pizza?" People are likely to write in "Yes," or "No," but they aren't providing much more information that you can code. A better open-ended question might be, "If you could eat only one food for the rest of your life, for every meal, what would it be? Why would this be the best choice?" People are likely to give a variety of responses and a great many different reasons for these responses, which you can then code and categorize.
