A researcher has been requested by a national day care provider to survey a sample of parents who use their centres to determine their satisfaction/dissatisfaction with the provision of day care. The following questions are included:

1 How many children have you?

2 What are their ages?

3 How long have they attended these centre(s)?

4 Do you get a government rebate to help with the cost?

5 What do you like best about the centre(s) your children attend?

6 Which day care centre(s) do they attend?

7 What do you like least about the centre(s) they attend?

8 What contact have you had with their carers?

9 How much per week does it cost, less any government rebate?
(a) Do these questions constitute a structured interview, a semi-structured interview or an unstructured interview?
(b) Is the sequence arranged correctly?
(c) Should the interview schedule be mailed out, conducted over the telephone in the early evening when both parents are at home, or face-face at the centre as parents pick up their children? In relation to this you may wish to determine what sampling strategy to adopt.
(d) Which questions may require probing by open-ended questions. Design some probing questions.
