A community organization (nonprofit) went bankrupt.During the deterioration of the $10 million dollar organization the following issues were disclosed:

1. A lack of reporting on the uses and impacts of grant/donor funds that were designated for a purpose/program.

2. Poor accounting of how dollars were spent and co-mingling of funds that were for different purposes.
