Cause to fails machine to boot

Assume You have a boot disk under svm, the machine fails to boot & remains in ok prompt? What could be the probable cause?

a)      Possibly issue with /etc/system file

b)      Root file system corrupted

c)      Wrong entries in /etc/vfstab file
