What message does it send when the culture encourages those involved to view mistakes/problems as opportunities for improvement rather than as reasons to blame or punish? I currently work for a Japanese company and have experience quality failure in one product for about 6 months, but for the organization it was hard to accept that the root cause of quality was coming from them especially when Japanese companies are well known for excel in quality of their product. After numerous suppliers Quality scorecards for all the board level components of the product, the company find out that one of the suppliers in China was providing counterfeit components. Japan made changes to their processes and adopt to use the (10-point quality and inspection process) that is currently being used in our Houston factory. We were able to gain more business and sales and customer trust in our product with this small improvement from our team in Houston."
