Discuss a recent case (within the past 3 years) that resulted in significant losses in human lives and/or material resources (money - time - building, etc.). The case should be a result of engineering error(s) which was known or should have been known to the engineer(s) prior to the accident. You must have a section at the end of your report expressing your opinion which should discuss the following questions:
1. Who is responsible?
2. What is the punishment if any?
3. How could the accident have been prevented? The report should be done by each student. The report should include a cover page and 3-4 single-spaced pages.
