1. Patients will see numerous physicians over time and may make several appointments in a given day.
2. Patients will use various insurance plans over time and a history of plans must be maintained with patient records.
3. The physicians need to be associated with one or more areas of expertise.
4. Patients will attend numerous patient education classes over time.
5. Each appointment must have specific details as to the reason for the appointment and related disposition(s) from that appointment (diagnoses, prescription, and referral).
6. Each appointment needs to be billable to one or more insurance plans, depending on the number of plans associated with the patient.
7. Each patient will have at most one billing address associated with him/her.
8. Each insurance plan will have at most one address associated with it.
