Working for a data gathering company, you are asked to create a webpage to gather information for a national survey. The survey will gather personal usage information on beverages (alcoholic/non-alcoholic), OTC painkillers, smoking and finally usage of cannabis.
Your webpage should contain an image carousel like picture above to rotate images of the products contained in the survey and at the bottom of the page a privacy disclosure statement that will disappear once the privacy agreement is accepted. Also, the submit button should be initially invisible, but once the privacy agreement is accepted it should become visible.
The survey should be designed with the following required fields: Birth Year, State, Race [White, Native American, Black, Hispanic, European, Asian], Residency Status [Citizen, Resident, Other]. When the submit button is clicked, all validation checks should be performed programmatically.
Also, for every item being surveyed there should be a minimum of three questions/statements to be answered.
