The paper must include an introduction paragraph, two or more body paragraphs, and a conclusion paragraph.
The introduction should briefly describe or define the topics. You may briefly state which studies you will describe in the body here, for example Milgram's study of obedience.
The body is where you will answer the questions. Describe studies (minimum of 2) that had ethical issues that failed to protect their participants' welfare.
Then you will describe how you will protect your participants' well-being, such as IRB, informed-consent form, voluntary withdrawal, and confidentiality, and debriefing if necessary.
The conclusion paragraph summarizes the entire paper and is similar to the introduction paragraph but does not have any of the answers to the questions.
