Why is health care only privilege for one section in society

Problem: Social inequality works to create disparities in this issue because individuals surviving with an income in the lower end of the pay scale are not given equal opportunity to achieve and maintain good health. In many instances wealth really does equate to health. (Underwood, 2014) Even outside of healthcare you see that with food sales, it is more expensive to eat a healthy diet.  What is the root cause of this? I see it on social media often. Someone will post a video showing how someone has poor oral hygiene, or something similar, and the comments are: "why did they let it get this bad?", "why didn't they just go to the dentist/doctor?"Health care is a privilege for many in America. Why is health care only privilege for one section in society and how can this problem be fixed?
