Problem

Josie is 55 and has younger onset dementia (YOD) from Huntington's Disease and because of this she is living in the memory support unit of an aged care facility. She is married and has two teenage children. Today is her birthday and her family had taken her to the park for a party. She has arrived back at the nursing home at 5pm and appears confused, upset and agitated. Her husband has stated that she would not sit and talk while out, preferring to wander around the park on her own. He stated that on several occasions he needed to redirect her back to everyone as she was wandering away and when he asked her where she was going she stated that she was "Going home". He stated that she had become quite angry when he had told her that she needed to remain at the park. After her husband leaves Josie continues to wander around the unit and continuously asks staff to open the door as she needs to go home and cook tea for her children who will be getting off the bus shortly and will be frightened if she is not there.

You notice that Josie is looking a bit tired and is becoming unsteady on her feet as she walks around the unit and may be at risk of falling. What would you do to minimise the risk of Josie falling and getting hurt.
