Behavior Change Assignment

OVERVIEW

In this class we will focus on lifestyle behaviors that impact health and wellness in the short- and long-term, identifying health promoting and health damaging behaviors and discussing ways to promote well-being. As a way to help you apply concepts to "real life°. this set of assignments is meant to provide you with the opportunity to monitor health behaviors, identify an area that could be improved and form behavior change goals. Once you receive feedback on your goal and planning, you will implement your goal, monitor your progress towards goal achievement. and reflect up on the behavior change experience. You will also identify physical and/or psychological benefits associated with your chosen health behavior, applying course concepts.

In part 1, you identified an area of change, created a goal, and developed a plan. Now, you will enact your behavioral goal, evaluate your success, and reflect on the process.

PART 2 DIRECTIONS:

1. Attempt your behavior change goal for at least TWO weeks. Hopefully you've been attempting this throughout the semester, but if not, now's the time to start.

2. Monitor your behavior(s), keeping track of when you successfully performed the behavior and what influenced it. On days that you were unable to accomplish your goal. make notes about what else was going on. You may track this using the calendar posted or using the logs from he first assignment.

3. Reflect on your behavior modification and complete a report on your behavior modification using the guidelines below.

WHAT TO TURN IN TO MOODLE FOR PART 2:

1. Monitoring logs, documenting successes and/or failures in goal achievement.
2. Written reflection of the behavior change experience.

Behavior Change reflection

1. Restate your goal. If you needed to make modifications to your SMART goal from the first assignment, make sure your goal statement reflects these changes. Your goal will be re¬evaluated according to the SMART criteria.

2. Summarize briefly why you wanted to make this specific behavior change.

3. What recommendation does your goal help you meet? Explain the recommendation (cite your source) and how your behavior change goal helps meet the recommendation (or bring you closer to meeting that recommendation.

4. Using your monitoring reports, briefly summarize how successful you were in implementing your goal, that is, how often were you successful (or not successful) and how did your behaviors compare with your intention?

5. Reflect on the process of implementing your goal and what influenced your success (or lack of success) and describe your experience. If you achieved your goal, what influenced your achievement? If you didn't achieve your goal, what do you think happened? Consider the following:

• Barriers to performing your goals and how you managed them (or could have managed them differently)
• How easy or hard it was to implement the goal
• integrating your behavior into your day
• Supports or facilitators to goal achievement

6. Review how you initially rated how easy/hard you thought achieving this goal would be. Based on your experience, do you agree with that rating you initially gave? Why or why not?

7. Reflect on your use of motivations and rewards. How helpful was it to have a reward or use motivational strategies in your behavior change experience?

8. Finally, what might you do in the future related to this or another behavior change goal? For example, if you achieved your goal, do you have other goals in mind or will you continue the current behaviors for awhile longer? If you didn't meet your goal, how might you modify your goal to be more achievable?

NOTE: You are not graded on whether or not you successfully changed your behavior or implemented a goal. Many of you may not be 100% successful, but going through the effort and reflecting on it are the most important parts of this assignment. If you were not successful talk about why you had trouble with goal implementation and what you could do differently in the future.

Your reflection should be at least three pages., using 1" margins, 12pt or 11 pt font. Double space your reflection. ASSIGNMENTS MUST BE SUBMITTED AS WORD DOCUMENTS.
