Her inflated self-esteem, less need for sleep, poor judgment and poor impulse control as a result of her promiscuous behavior, increased talkativeness, flight of ideas and racing thoughts, irrational cognitive processes with grandiose notions, and delusional thinking are some of these symptoms. Following these periods of "creativity," the client describes feeling useless, lacking drive, having trouble getting out of bed, and losing interest in activities she usually enjoys. To prevent adverse effects, the client will begin taking 25 mg BID of lamotrigine and be gradually titrated to the desired effect. Additionally, the individual will be sent to psychotherapy. Lamotrigine was selected because weight gain is uncommon and since the FDA has approved it for the treatment of bipolar illness, despite the client's previous concerns about gaining weight while taking her prescribed drugs. Weight gain is an unwanted side effect that several atypical antipsychotic drugs and other mood stabilizers have been known to cause. The client will be informed about the typical side effects, including rash, nausea, and dizziness, as well as the possibility that it may take several weeks for her symptoms to improve in order to ensure drug compliance.
