Problem: Needs Statement. Youth transitioning out of foster care face significant challenges related to housing instability, family disconnection, and economic insecurity. According to the National Foster Youth Institute (2024), approximately 20% of youth who age out of foster care experience homelessness at some point after leaving care. Additionally, many foster youth lack supportive family relationships and face barriers to education and employment, increasing their risk for poverty and housing instability. Research suggests that comprehensive programs that combine housing assistance, family engagement, life-skills development, and case management can improve long-term outcomes and reduce homelessness among former foster youth (U.S. Department of Housing and Urban Development [HUD], 2024). Therefore, there is a demonstrated need for a Family Unification and Housing Stability Program that supports transitional-age foster youth in Contra Costa County as they move toward independent adulthood. Goal 1 Increase housing stability among transitional-age foster youth in Contra Costa County through the Family Unification and Housing Stability Program. Objective 1 By June 30, 2027, 80% of program participants will obtain and maintain stable housing for at least six consecutive months through housing navigation services, landlord partnerships, rental assistance referrals, and case management support.
