Problem

Can anyone help me with the abbreviation and what they mean on this surgical report?

John G. is a 40M (was on ASA81 for blunt cerebrovascular injury, last dose was sometime last week) who initially presented s/p MVA 1/17/23 with ETOH intoxication. F/t/h on head CTA to have R>L temporal lobe contusions, scattered SAH, small bilateral SDH, scattered pneumocephalus, and extensive basal skull fractures extending into bilateral carotid canals a/w luminal irregularities and ?blunt injury. Underwent middle fossa craniotomy with skull base defect repair with ENT on 1/27.

Patient had surveillance HCT (2/19/23) after discharge from rehab that demonstrated an increase in L SDH with new hyperdense components and MLS. Was recommended to undergo a repeat left craniotomy but canceled per wife's request as she did not want to risk compromising ongoing recovery. Another surveillance HCT (3/4) demonstrated slight increase in size and density of L SDH with further MLS. The patient now presents for L craniotomy for SDH evacuation.
