Question evaluation:  The questionnaire begins by asking how the students would rate the professor.  This is an effective first question because it awakens interest and motivation by asking a direct, simple question about the professor's competence.  Also it is a question that virtually all students would have an opinion about.  As far as further evaluation of the questions asked, I would say that question #2 could be improved by using a scale of 1 to 10 instead of a simple yes or no question. Example:  instead of 'Does the professor grade fairly?" I would ask "How fairly does the professor grade using a scale of 1 to 10, 10 being very fairly and 1 being not fairly at all.  Finally, question #5 is too vague and ambiguous.  I would word the question to read "Comment on the kind of class the professor teaches."  That way the students could write in the general characteristics of the class such as formal, informal, etc.  

Instrument Format:  I would say question #6 should actually be the second question asked following the rule of the questioning process being simple questions first, then moving to the more complex.  Next, using the funnel approach, I would put the questions in order of question #5 being before numbers 4 and 3 because this would follow the rule of general questions first, then moving on to the more specific.  In short, I would change the order of the questions to 1,6,2 (a-g),3,4,5 instead of 1,2,3,4,5,6.
