Case Scenario: Infant with Acute Gastroenteritis, Mild Dehydration

Course in the Ward:

On September 22, 2022, you are assigned in the Pediatric ward of the hospital and assigned to Joseph Andrew Camintal, who is admitted due to Acute Gastroenteritis. He just turned 11months last week. His father is John Jacob Camintal, a 34-year-old cyber analyst and his mother is Nathalie Drew Camintal, a 31-year-old stay at home mom. He is admitted due to the complaint of vomiting and loose bowel movement of 5x/day with color of greenish brown stool for the past week. This prompted them to go to the hospital for check-up/admission.

You are conducting a thorough health assessment on the patient and interviewing his mother Nathalie, about JAC's growth and development. Nathalie states that he is already about 9kgs now and is already trying to stand.

She remembers the time when she gave birth with JAC, the doctor said "Congrats it's a healthy baby boy!" He was 2.5kgs at birth, and gave a good loud cry. It was the happiest and most tiring day of her life.

At 1 month old was now able to lift his head a few moments at a time. He would try to step on to somethin gas if he can already walk. He had this strong grasp and would often hold her finger. He was so cute making this cooing sounds. When Nathalie would change his diaper, he would often stare at her. JAC likes it when his music box is played when his diapers are being changed. He usually doesn't fuss.

On his 2nd month, JAC was able to hold his head in the same plane as the rest of his body and when in prone position he is able to raise his head and maintain that position but cannot raise his chest. When Nathalie tries to place JAC in a sitting position, he can hold his head fairly steady, although it does tend to bob forward. Nathalie's cousin always says that JAC was a strong baby boy. During this time Nathalie notices that JAC now opens his hands unlike before that it was always closed. Now he coos a lot and cries when he is hungry or when he needs a diaper change .He is able to hold his small rattle now but drops them shortly after. Now when Nathalie changes his diapers, JAC would look at her face and smile at her. It would brighten up Nathalie's day even when she is dead tired.

On his 3rd month, Nathalie remembers that JAC can now lift his head and shoulders well off the table and look around when prone. When Nathalie tries to change his diapers, JAC would often try to grab her gold necklace. He also squeals when tickled by Nathalie. He is now able to hold his favorite small rattle without easily letting go of it. He often looks around when he hears his favorite music box being played. He smiles when he sees his mother approaching to feed him.

At 4 months JAC can now lift his chest of the bed and look around, turning his head from side to side. When Nathalie puts JAC into a sitting position he has no more head lag and can now support this head. Nathalie noticed that JAC can now bring his thumb and fingers together. It was really cute to look at. JAC is also now very talkative and responds by cooing and babbling. During this time JAC also like to roll over. That's why Nathalie and John decided to get a playpen for JAC. When JAC cries he is easily soothed when given his favorite rattle. It always does the job of keeping JAC calm. Since John is mostly at work, Nathalie is the main caregiver of JAC, sometimes JAC would prefer her mother over his dad. He would sometime cry when his father will be the one to change the diapers.

On his 5th month, JAC can now completely turn over. He can now straighten his back when held in a sitting position. He can now accept objects by grasping them with his whole hand. He can also reach and pick up objects without the object being offered. Nathalie noticed that during this time JAC likes to play with his toes. It was also the time when JAC said goo-goo. He would cry if you take his favorite rattle off his hand.

At 6 months, JAC is now able to raise his chest and upper part of his abdomen off the bed. He can seat momentarily without support but goes back to lying down in a short while. When His father plays with him and raises him way up and throws him up in the air, he would extend his arms like a parachute. It was so funny that Nathalie and John would laugh. JAC can now hold objects in both hands and will usually drop one toy when a second one is offered at the same hand. During this time, he holds a spoon and tried to feed himself but it usually is a messy situation. During this time, he can now sit steadily when being bathed and plays with his bath tub toys. He has his new favorite rubber ducky. This time is also the time when his teeth start to appear. He is usually irritable during this time. That is why Nathalie bought a teething ring for him to chew on. It helps him calm down sometimes. He was quite heavy at 5kgs.

On his 7th month, JAC can sit alone with hands held forward for balance and can now transfer toys from one hand to the other. He likes imitating vowel sounds like oh-oh and ah-ah which always brings delight to Nathalie. He likes to pat his image in a mirror and now shows obvious fear of strangers.

On his 8th month, Nathalie can remember that JAC can now sit properly without any additional support. He cries when other people try to carry him or talk to him. This was a difficult time for Nathalie because she was the only one who JAC is not afraid of.

On his 9th month, JAC was able to speak his first word "da-da", and John was so happy hearing it. He is now able to crawl. That's why Nathalie had to keep an eye on him because he would always crawl. Sometimes Nathalie would scold JAC for crawling everywhere and this would make JAC cry.

On his 10thmonth, JAC was able to grasp things using his fingers and pick up small objects. That's why Nathalie had to clean all small objects. During this time, he always says "No". He can now stand up with support but cannot sit back down. He likes the game peek-a-boo. And listens when called by his name.

During this 11th month, He is still trying to move about in his crib and tries to hold to the crib rails. He keeps saying "No" during this time.

In the past week JAC has been feeling sick, he hasn't been drinking his milk as much and he is always vomiting, three times a day and has loose bowel movement. Nathalie had to change his diapers 5 times a day with watery stool. She also noticed that the color of the poop is greenish brown. Baby JAC is starting to feel weak and doesn't interact much and cries and is irritable most of the time.
