Feedback form

Feedback is an essential part of a good presentation. You should always welcome feedback following any public delivery of information. This is how you will improve.

For the presentation and speech that you have prepared, also prepare a feedback form that would best provide you with the information you would want to know in order to improve your presentation for next time.

You can refer to the examples of different types of feedback forms provided in the attached link, or you can research more if you wish. Using these examples, design your own feedback form, focused on your presentation.

There should be at least seven questions. You can decide whether you ask open ended questions, or closed questions; and whether you use a ‘circle the number' format, a tick box format, or an open paragraph format.

At the top of the feedback form, ensure that you provide a paragraph explaining why the feedback is appreciated and what it will be used for. You can decide whether it should be anonymous or not.

Minimum length: One A4 page
