Practice of a chronic complex client


Problem: Apply semester 1 program concepts and course concepts from weeks 1-4 to develop a plan of care for the client.

Instructions:

Choose a client from experiential and further develop or from your nursing experience/ practice of a chronic complex client or make a scenario of a client with chronic complex disease.

Apply your first semester courses of evidenced-based practice, pharmacology, health assessment, and pathophysiology

Part 1: Plan Of Care Approach (60%)

  • Brief Outline (max 5 sentences)

  • In a simple paragraph outline your client's diagnosis , medications, symptoms, and vital signs

  • Review the assessment data collected.
  • From this information identify and create a list of areas of concern/symptoms for your client

  • Prioritize - List with rationale (15%)
  • Prioritize the grouping of data collected from the highest to the lowest order of priority with rationale
  • Identify the main priority of care
  • Judgments and inferences (ways of knowing) (25%)
  • Look back at the data you collected.
  • Remember during data collection you are making inferences/judgments.
  • Ask yourself how do I know this?
  • What data did I collect to support my inference or judgment?
  • State a goal of care based on priority (10%)

 Part 2: Creativity (30%) 

Each paragraph represents the culmination of your understanding of course concepts. Integrate several aspects of course concepts in a cohesive manner to demonstrate your understanding. Creativity is the use of original ideas that are innovative and presented in a unique manner.

 Part 3: Academic writing and APA formatting (10%)

APA Formatted:

  • APA format with a clear and well-developed introduction, body, and conclusion
  • 4 pages, Word processed double spaced responses in APA template, times new roman, 12 font, 2.5 cm margins
  • A minimum of 3-course resources must be utilized and referenced within paper (in-text citation) and in the reference list
  • Title page
  • Template completed including in-text citations/ references, title page and reference page
  • Reference page
