Assignment Task 2: Major Activity - Written Report and case plan

Develop a case management profile for two different potential clients who are requiring case management. The profile should include:

a) A brief description of your agency and role to set the context for why/how you are providing a case management service

b) Background information (The reasons why the client requires case management)

c) Risk factors that may apply for client and/or case worker

·         Medical history

·         Current physical wellbeing, including and psychological conditions

·         Employment status

·         Accommodation

·         Familial relationships

·         Culture

·         Specific communication needs i.e. interpreters, literacy, access to technology

·         Other information you consider relevant to the case study

Based on the client profiles you have created, make an assessment of each client and develop a case plan. As the assessment and planning stages of CM are collaborative processes, you should provide an overview of how you would complete an assessment and how the case plan would be created for each scenario.

The report should include, for each client:

What you would need to consider when allocating the case to a worker (i.e. experience, workload and geographical location of worker)

An overview of how you would complete the assessment and work with the client to create the case plan (i.e. how was the client included in the creation of the plan)

A clear understanding of what actions will be taken for each client

·         What services can your agency provide to the client.

·         What other services would you involve and why

·         How would you communicate with your client and the other services to minimize confusion and overlap.

A case plan that addresses the following:

- What are the goals? Are they realistic? Have you considered immediate, short and long term needs of the client and other relevant parties?

- Who/what resources might be needed to assist the client in reaching their goals?

- What are the timeframes?

- How will it be monitored?

- How will it be evaluated?

- How were appropriate cultural considerations integrated into the case management planning process?

- How would you get client feedback their progress and services? Why is getting this feedback important?
