In obstetric care, patients often ask about costs related to procedures like ultrasounds, inductions, or cesarean deliveries. Each of the following questions is important to a patient to help them financially plan and understand the care they will receive. The question "How much would the procedure cost because I have not yet met my deductible?" is important because patients need to know what they will expect to pay out of pocket and what the contracted rate is for the service. This helps patients plan financially and can reduce some of the financial stress during pregnancy. The question "Do I need preauthorization" is significant to a pregnant patient, such as the ones that I care for in the fetal care center, because tests like MRIs or fetal echocardiograms can be costly and if the insurance company requires a preauthorization and it is not approved, the patient is left responsible for the bill. "Is there a facility charge in addition to the physician charge?" comes up often in obstetric care because the hospital I work for has charges for services to be performed that do not include separate provider charges. An example of this in my clinic is the cost of a nonstress test. There is a facility charge to have the test performed and for the equipment required and also an additional charge for the physician to interpret the nonstress test. Finally, the question "What other charges would be incurred" is crucial for my patients to understand because there are often additional fees from anesthesia or labs. Knowing this upfront helps patients plan for total costs.
Procedural expenses are based on negotiated rates between healthcare organizations and insurance providers. Costs typically include professional fees, such as a physician fee, facility fees, such as hospital or clinic fees, and ancillary services such as labs or imaging. Insurance coverage, deductible status, and out of pocket maximums further determine what the patient ultimately pays. When patients understand their financial responsibility and feel as though the billing process has been fully explained to them, they are more likely to make payments on time and in full. In high-risk obstetric care, trust is essential because these special pregnancies require multiple visits, ultrasounds and sometimes procedures. If patients feel blindsided by unexpected charges, they may delay payment, which leads to revenue loss for the clinic and hospital system (Bernstein & Crowe, 2024).
