Assessment Task - Business Report - Initial Interim Findings of Incident Causation

You have been asked to provide an interim report to senior management regarding the case that has been unfolding during the teaching period. The management team understand that a full and comprehensive incident report will not be forthcoming for many months, but want some initial understanding of basic causes (not root cause). With reference to theories of accident causation, analyse the basic cause of the incident. For the purposes of this initial report, you should confine your analysis only to data and evidence you could gather on your initial site visit.

As such you will not consider systems, procedures, or organisational failures in this initial interim report (this will come in the next report). As your report will go to the board, you need to ensure your analysis is evidence based and defensible. As such information presented and conclusions drawn should be informed and supported by scholarly reference material, with intext citations, and a reference list at the end. You must write your report independently (as verified by turnitin) but as is the case in professional practice, you are encouraged to discuss relevant parts of the case and incident management with your peers (in this case with your fellow students who are in the role of your OHSE peers in other parts of the same organization).

Your report should have the following headings:

1. Brief description of incident;

2. Limitations of report;

3. Methodology (including actions taken on initial site visit in relation to decision making, communication, compliance, and worker wellbeing);

4. Evidence (list of evidence you have collected to date including photos where relevant);

5. Timeline of events;

6. Application of theory/model/framework of causation and initial interim findings (including brief description of, and justification for, model chosen);

7. Appendix - Your appendix should include a brief (around 400wds) 'reflective practice' discussion outlining the aspects of the case that influenced your professional judgement and any associated challenges/learnings when you undertook your virtual initial site visit.

During your discussion please note how you used peer collaboration and other learning activities on LEO to help you throughout the process. Please include at least two examples of your peer discussions and LEO activities/quiz findings (cut and paste from LEO) to support your statements.

Purpose: This task aims to assess your ability to apply practice skills of collaboration and communication in the way you manage first response to an OHSE incident. Collection of evidence and application of a model to analyse causation of an OHSE incident. Your ability to critically reflect on your actions and judgements when responding to OHSE incidents will also be assessed.
