Problem:
His parents kicked him out of their home and he was briefly homeless, Xander also dropped out of high school. He finally obtained a High School Equivalency Certificate by completing GED testing, and now he is considering enrolling in community college to pursue his interest in graphic design. He is feeling a bit unsure about balancing a school schedule while continuing to work, but he wants to keep this job since he has worked there for a few years, and he knows that they value his work. He has been struggling with depression for a long time (at age 14 he was diagnosed with clinical depression without mania/mood swings) and, despite looking into college, is not optimistic about the future.
Six months ago, Xander's roommate called 911 after finding him unresponsive next to empty bottles of pills and alcohol. After two days in the ICU, Xander was transferred to a psychiatric unit where he admitted that he had attempted suicide. As part of discharge planning, he was connected to community mental health services. Xander usually attends appointments with his case manager and therapist, but doesn't really see how this is helping, so he sometimes decides to sleep in and misses a scheduled appointment. He doesn't think that this is a big deal, but his therapist is concerned that this behavior is impacting their therapeutic progress and questions Xander's commitment to getting better.
Xander works about 25 hours each week at a local coffee shop. He is a natural at finding what he needs when he is short. When he needed a low-rent apartment, he found a roommate to split the cost. Due to missing work when he was in the hospital, he did not have enough for his share of the rent, but he knew he could rely on his sister to loan him the difference and he has been paying her back. He does not have a car but was able to identify transportation options so that he can always get wherever he needs to go.
Xander knows and gets along with many people who are regulars at the coffee shop. Still, he terms most of these people "friendly acquaintances" rather than friends as he doesn't spend any time with them outside of the coffee shop. He enjoys solitary activities such as reading and drawing in his spare time. Some of his drawings are on display at the coffee shop where he works.
Xander says he misses being part of a faith community but hasn't been able to figure out how to find a church community that will accept him as he is. Without a home community, he feels that he is unable to practice his religious traditions. He states: "I just feel emotionally exhausted all the time. I'm tired of people looking at me strangely when I go into the restroom. Tired of people still insisting on using my birth name and calling me a 'she.' Tired of hearing on the news that basic medical care is a privilege. Tired of parents complaining that their children going to school with people like me is against their values. I feel like I don't belong in this world. It's easiest to just sleep and try to shut everything out."
