Cancer Clinic Case Study

You are a project manager working on a project at a local hospital for their cancer clinic. There are 2 parts to the project. The clinic has been using paper based reporting and are now going to move all files on-line and give each doctor a computer to use. There will no longer be an option to use a paper filing system. The second part is the clinic would like to build and introduce a new system that will allow patients to complete a series of screening questions on pain levels, mental and financial health prior to seeing their doctors. The system will involve a bank of touch screens where patients can log in using their health card number and rate themselves on a series of metrics. The results of metrics will then be sent to the patients doctors, flagging any issues that may be of a significant concern (for instance, if the patient is displaying signs of depression). This allows the patient to share this sensitive information once and have it relayed to all health care professionals. Patients have also shown they are more likely to rate themselves accurately using the computer interface rather than bring up these types of issues to their doctors.

Approximately 500 patients visit the clinic daily. Patients typically see between 2-5 doctors per visit depending on their stage of treatment and may have appointments once per year or multiple times per week. They may be at the clinic for an hour or several hours per visit. There are a number of volunteers that helps to support patients at any given time at the clinic (providing activities, snacks, coffee etc).

Currently, patients must report to a main reception desk upon arrival and then they are entered into a check-in system which indicates to each of their doctors that they are available for their appointment. Check-in typically takes approximately 20 minutes and the intention is that patients will complete the screen prior to checking-in. Patients are assigned a waiting area and are picked up by each doctor from that area. This project will also involve automating the communication to the doctors so that they can see where a patient is and who they are scheduled to see next at any given time. Doctors will be using ipads and computers for all communication and record keeping.

It is clear to you on the benefits of doing this project. You have heard rumblings that the older staff are not comfortable with electronic files and communication as they have their own paper system in place.  They are also concerned with having less time with their patients. Many of the patients are above 65 years old and many have indicated they do not use any technology at home. There are approximately 30 doctors in the clinic, plus administrative staff and volunteers (who rotate frequently).
