What medications are you taking?

• Before the interview research each of the medications. In your paper include the reason for each medication, the drug classification, the route of administration, the dose, and possible side effects/adverse reactions.

For the following questions, paraphrase the client's answers for your written paper.

A. How long have you taken each these medications?

B. Do you know why you are taking these medications?

C. Who told you about the medications and why you need to take them? Were you able to ask any questions about the medications and if so were they answered so you understood what was said?

D. How do you feel about taking these medications?

E. Are you taking other medication purchased "over the counter" such as in a drug store? If so, what is it and why are you taking it.

F. How and when do you take your medications?

G. Do you have any difficulty taking the medications? If so, what happens and how do you deal with any difficulties.

H. Have you ever felt any uncomfortable feelings and/or body reactions to taking these medications? If so, what were they and how were they handled?

I. If you had the above reaction, did they take you off the medication and/or replace it with another medication? Were you told why this happened?

J. Do you have any reason not to take the medications?

K. Have you ever not taken the medication? If so, what was the reason?

Case study:

Molly Jenkins is a 25-year-old female with no past medical history. She has been married for a little over 3 years and has one child, a girl, age 4 months. Molly is a full-time student with goals of becoming a lawyer. Her spouse currently works in pharmaceutical sales and travels often. Molly was brought to the ER with delusional thoughts and witness seizure activity.,

BP 190/100, RR32, SPO2 90% on RA, HR 130, BG 1100,
LABS INR 0.5, BUN 68, CR 1.25, UA positive for UTI.

Toxicology screen negative.

Mrs. Jenkins was recently diagnosed with BIPOLAR, HTN, DMII and afib after the birth of her child. She complains of back pain and not being able to sleep at night. She is also having intermittent chest pain and pressure. She has no prior history of taking medications besides birth control pills. Molly takes OTC supplements for energy and smokes recreational marijuana to help her relax. Molly admits to not taking medications as prescribed, she does not believe she needs all of them especially the ones for depression. Molly states she does not feel depressed.

Molly's mother was diagnosed with Bipolar and depression at the age of 26 when Molly was a child.

Medication list:

• Digoxin 5mg daily
• Novolog Insulin 8 units BID and 10 units of NPH once daily
• BG checks TID
• Lithium 150mg bid
• Diltiazem HCI 50mg TID
• Sertraline 50 mg daily
• Atenolol 50mg once daily.
