Reflecting on your learning is an effective practice that helps you become a better learner. It also helps me see where you might need some additional support. In this activity, your job is to talk about your own learning. Nursing process is a systematic problem-solving process that guides all nursing activities. It is the type of thinking and doing nurses use in their practice (Treas, Wilkinson, Barnett Smith, 2018). Critical thinking is a combination of reasoned thinking, an openness to alternatives, an ability to reflect, and a desire to seek truth (Treas, Wilkinson, Barnett Smith, 2018). Documentation and reporting in nursing are used for communication, continuity of care, and quality management. Documentation is the proof that nursing interventions were performed, and this is how nurses communicate with other nurses and the rest of the healthcare team. Reflect on your past experiences and the information that you learned in this module. Think about the different methods of charting along with use of the electronic health record (EHR). If you have had previous charting experience, which of the formats of charting do you feel most comfortable with, and why? Has this module changed your ideas on documentation at all? If so, how?
