Determine the need for additional nutritional support


Problem: An 81-year-old woman was admitted to the hospital 3 days ago because of hypotension, depressed sensorium, and urosepsis. Five weeks earlier she had perforated gastric ulcer and sepsis. Her postoperative course necessitated prolonged stays in the hospital and nursing facility. In the current hospital stay, she responded to IV fluids and antibiotics and is now alert and cooperative. For the last 2 days, she has been on a regular diet with polymeric oral nutritional supplements between meals. Nurses notes indicate that her nutrient consumption ranges from 25-75% of meals. Weight is 142 lb, down from 152 lb 3 months ago. BMI is 21.6. There is a noninflamed surgical wound, which is being allowed to heal by secondary intent.
