After evaluating three service oriented organizations, select one for which you will complete your service project.

This service project should take between 15 and 20 hours to complete. Outline your service project, written in 500 -750-word essay form rather than as a list of responses.

We discussed integrating faith in our work and the qualities of faith affecting our leadership abilities. You now have found and evaluated three community-based organizations. In this Module, you will select one of these to complete your service project (15-20 hrs.). In your assignment this week, you will be providing details about this organization and ideas on how it will help develop your skills as a servant leader.

Please start utilizing the Project Plan Table (located in "Additional Material"). Please fill out each question/statement as detailed as possible. You will be using this throughout the weeks, and it is due in Module 7.

Arizona State Veterans' Home (ASVH) Transportation Department

Volunteer Service Project

Volunteer's Role: Assist veterans, who are residents at the Arizona Veterans Home, go to and from appointments at the Veterans Administration (VA) Hospital.

Time Commitment: Volunteers sign up for the hours they are available depending on the day. Typically, the time commitment ranges from 3-5 hours once a week.

Responsibilities:

Provide references, resume?, and cover letter outlining volunteer and/or work experiences appropriate to volunteering in the Transportation Department at the Arizona State Veterans Home (ASVH).

Complete the volunteer application and secure fingerprint clearance. If the volunteer already has documented finger print clearance, she or he should submit a copy along with the application.

Attend training sessions provided by the volunteer coordinator, duty nurses, and caseworkers.

Contact the volunteer coordinator by telephone the day before your scheduled visit to confirm the date and time of the veteran's appointments.

Arrive 30 minutes before the first scheduled pick-up and check in with the volunteer coordinator.

Confirm the details of the appointment with the ASVH unit duty nurse.

Pick up and review the Routing Sheet from the unit secretary who set up the VA Hospital appointment. It should contain:

• Resident's name;

• last four digits of his or her Social Security number;

• name and location of clinic or department (e.g., eye, audio, x-ray);

• name and role of person with whom the appointment is scheduled (e.g., physical therapist, nurse practitioner, lab technician, doctor);

• description of required medical assists or equipment (e.g., wheel chair, foot rest, full oxygen tank); and

• Contact information for unit nurse and transportation department.

Pick up but do not review the Routing Sleeve, which contains HIPPA protected and confidential information regarding the veteran.

Secure required equipment listed on Routing Sheet and sign out the veteran from his or her ASVH unit.

Meet the veteran in his or her unit, making introductions and explaining why the volunteer is there, and confirming the details of the appointment.

Assist the veteran as appropriate depending on whether he or she is using a cane, walker, or wheel chair. If the veteran is using a scooter, walk along side to prevent him or her from becoming disoriented or lost.

Make sure the veteran is signed in upon her or his arrival for the VA Hospital appointment.

Depending on instructions, either wait for the veteran or return for the veteran at the end of the appointment.

Make sure veteran has all the required documentation/prescriptions before assisting her or him back to the ASVH.

Return the veteran to the proper wing and notify the unit nurse and secretary of his or her return. Provide any instructions or paperwork received from the VA Hospital appointment to the unit nurse, whether follow through is required or not.
