Present your data and assessment findings to the healthcare professional. In collaboration with your community healthcare member, choose one prevention strategy listed in Part V and develop a community diagnosis and an intervention plan that considers assessment findings with a focus on determinants of health, available resources, and the range of activities that contribute to health and the prevention of illness, injury, disability, or premature death.
