Assignment Task: Medication errors happen all the time during administration at the hospital. A lot of technological advancements have improved numbers but the human aspect of errors has yet to be addressed. I propose to use a symbolic barrier when passing medications such as a yellow sash to ensure every hospital staff realize not to interrupt a nurse that is in the middle of a medication pass. This is cost effective, different than a technological change so it will be easier to implement, and hits the main point that the nursing field is always a distracting environment which will lead to mistakes.

1. Define the role of leadership and change management in addressing the medication administration errors in the hospital.

2. Propose strategies for communicating and collaborating with the nursing staff to improve outcomes associated with medication administration errors in the hospital.

3. Explain how state board nursing practice standards and/or organizational or governmental policies guided the development of decreasing distraction and utilizing symbolic barriers during medication administration passes.

4. Explain how using symbolic barriers during medication passes in the hospital will improve the quality of care, enhance patient safety, and reduce costs to the system and individual.

5. Explain how care coordination be applied in addressing the problem.
