Hospital Follow-up

Currently your practice is receiving hospital reports by fax or mail. Your practice has no process in place regarding what to do with these reports when they come into the practice. Sometimes the reports are sent to the nurse, sometimes to the Physicians, and typically, these reports may sit in the Physician inbox for 2-3 days. In addition, there is no standard process for receiving reports back from the When the reports come into the practice, the medical records staff scan into the EMR and then forward to nurse and physician in-basket for review on the same day they are received unless received after 4:00PM (these will be scanned or distributed early AM of next day).

Goal: To establish guidelines for management of hospital follow-up reports and timely, efficient coordination of care provided in hospital.
