The Medical Center of DeVry wants to automate its admission process. It wants to be able to add a new admission center, including demographic, insurance, and assessment information, and it also wants to have a checklist of admission paperwork generated. The Center's leaders also want the ability to update resident records. Demographic information includes name, address, gender, birth date, religion, marital status, Medicare number, and Social Security number. Insurance information includes payor, case number, group number, and authorization number. Assessment information includes assessment date, evaluator, current diagnosis, and allergies.

Associated with the admission process is the maintenance of a listing of physicians associated with each patient admission. Physician information includes physician name, address, city, state, zip code, phone, cell phone or beeper number, fax number, affiliated hospital, and state physician ID number.

STEP 1: Create an Entity Relationship/Data Model Diagram

Create a data flow diagram, including external entities.

On the File menu, point to New, point to Software and Database, and then click Database Model Diagram. On the menu, select Database. Below Database, click on Display Options. Select the symbol set you want to use and other table and relationship options, and then click OK. Once you start a database model diagram, the real work of refining it begins. You can add tables, columns, relationships, and data types to define your model, based on scenario requirements.

STEP 2: Add Tables
From the Entity Relationship or Object Relational stencil, drag an Entity shape onto the drawing page.

STEP 3: Add Columns
Double-click the table to which you want to add a column and then in the Definition section of the Database Properties window enter a Physical name for the table. Next, change the Categories selection to Columns. Click the Physical Name field of a blank line, and then type the name of the column. Press the ENTER or DOWN ARROW key and repeat for each additional column that you want to add to the table.

Add Relationships

Click the Relationship shape on the Entity or Object Relationshiptab. Position the Relationship shape over the center of the parent table so that the table is outlined, and drag to the center of the child table. When the outline appears on the child table, release the mouse button. Both connection points appear red, and any primary keys in the parent table are added as foreign keys to the child table. Define the relationship by editing its properties.
