1. The first part of the assignment is to make a graph showing the productivity of each one of your staff members who perform the Release of Information function within your department. You will see the table within the assignment lists each employee and the number of requests they complete

How will you now convert this data into a graph? You might want to enter the data into an Excel spreadsheet. You could enter the name of each coder and the number of requests they complete each week and then convert this into a line graph or even a bar graph. This will provide a visual tool for you to see the productivity achieved by your four employees.

2. Next you will create a flowcharton how the Release of Information function flows within your department. You may want to do some research on the Internet or read in your reading handouts to learn more about Release of Information.

Think about how the requests come to your department. Do you have customers dropping off their requests, receive them through the mail service or a combination of both? Who takes the requests if they are dropped off or who opens the requests if through the mail?

After the request is received are you pulling the patient's health record from the file room or do you have an electronic health record or does your staff have to go to off-site storage to retrieve the patient's record?

Do you copy the patient's health record or are you printing off the copies from the electronic health record? Do you mail the copies now to the requestor or do they pick the copies up or both?

3. Now look at the productivity of your staff and how the Release of Information flows within your department. Analyze the graph and flowchart. Are the four employees are performing equally as they complete the requests? Is there an employee who consistently falls behind in productivity compared to the others? What would be your plan to assist employees to achieve a more timely manner? What types of things may interfere with the flow of the function?
