Problem
Create a personal portfolio of your experiences in the practicum. All portfolio items must be submitted; therefore scan any paper items that may not be electronically stored. The portfolio should include:
• Professional goal statement.
• A counseling case study, with identifying information carefully removed, showing your counseling theoretical perspective, biopsychosocial assessment, a collaboratively constructed treatment plan, course of treatment, and discharge summary.
• Presentations within the agency setting or at conferences or professional meetings.
• Personal bibliography of books and articles read during your program.
The response should include a reference list. Double-space, using Times New Roman 12 pnt font, one-inch margins, and APA style of writing and citations.
