Chief complaint: follow up diabetes, complaint of lower extremity tingling and numbness. HPI: 68-year-old male presents for follow-up encounter to discuss test results from his annual physical exam and diabetes, with new complaint of bilateral lower extremity tingling and numbness. Today patient was asked to return to review worsening lab results regarding diabetes and renal insufficiency. Patient with past medical history of hypertension, hyperlipidemia, type 2 diabetes, gout, vertigo, and chronic renal impairment stage 1. Patient has new complaint of tingling to bilateral lower extremities. Patient reports the discomfort has been on and off for approximately two months. Patient reports he has not been taking his medication as directed and only taking the metformin once a day and the lisinopril every other day. Patient states he feels fine and does not think he needs to take all the medicines. Patient denies any increased thirst, hunger, or increase in urination. Patient reports he gets up once a night to go to the bathroom. Today patient complaining of bilateral lower extremity numbness radiating down both legs to the feet for approximately few months. Patient denies any trauma, fever, chills, or weight loss. Patient denies any history of back problems, or old trauma
