Child Admission Form
Our company runs premium athletic clubs and spas. In each facility, there is a large child care facility. In order for our members to utilize this both child must have an admission form on record.
Using Twitter Bootstrap, PHP as well as SQL Server, develop a Child Admission Form (see attached) that is reachable on all devices (desktop, tablet, phone and so on)
Every jurisdiction has specific requirements on additional forms that some children may be subject to be completed (attached supplemental forms).
Part of this job is to as well migrate existing form data from a a legacy web application.
Please evaluation Business Requirements (attached) and follow-up with questions for final proposal.
