Adults in rural communities with type 2 diabetes mellitus


Problem:

Adults in rural communities with type 2 diabetes mellitus (T2DM) face elevated complications due to limited healthcare access, provider shortages, and low health literacy (Boon et al., 2021; Yu et al., 2022). This project targets these individuals and will be implemented in rural healthcare clinics with limited DSMES access. The proposed intervention is a nurse-led, blended-delivery DSMES program that combines the R-D1D curriculum with culturally adapted telehealth and in-person coaching (Litchman et al., 2022). It aims to improve glycemic control, self-efficacy, and long-term health outcomes using digital tools and interprofessional collaboration (Kerr et al., 2024). Addressing this need is critical to reducing health disparities, as DSMES has been shown to significantly improve HbA1c and patient quality of life (Camargo-Plazas et al., 2023; Mannoubi et al., 2024). The intervention will be implemented over 12 months using the PDSA cycle, including staff training, stakeholder engagement, and iterative feedback (O'Brien et al., 2023).
